Our Mission

Our Mission is to provide optimal comfort and dignity to our patients through their end-of-life journey. We will achieve this by concentrating on patient and family centered goals with a focus not only on the medical needs but also by providing care for the mental and spiritual needs of our patients and families during their journey.

Our Caregivers

At Comfort Heart Hospice, our focus is to provide comfort, maintain dignity and enhance quality of life for all our patients. Hospice focuses on controlling pain and symptoms and is generally provided at the patient’s home but it can be separate residence or part of a nursing home. We value our commitment to having a positive, sympathetic, or friendly attitude toward our patients.

Our Goal

We coordinate all patient services; including the primary care physician, caregiver(s), spiritual advisor(s), families and friends, and the insurance company to ensure an all-inclusive approach to their welfare.

At Comfort Heart Hospice, our routine interdisciplinary teams, are composed of Medical Directors, Patient Case Managers, Registered Nurses, Hospice Aides, Chaplains, Social Workers, Bereavement Coordinators, and other professionals who are fully dedicated to providing unrivaled services. Our, hospice, closely teams up with each client to brand their unique and individual needs.

Opportunities

We offer wide-ranging hospice services, including pain and symptom management, emotional and spiritual support for patients and bereavement support for families.

Comfort Heart Hospice is licensed by the state of Texas, but is in the process of being certified by Medicare, and accredited by the Accreditation Commission for Health Care (ACHC). However, Comfort Heart Hospice has been providing community care services in the Dallas/Fort Worth metro area since 2018.
